Job Description

Health Equity Access Coordinator

HEAC is a position that is instrumental in providing access to patients at Grady Health System. The HEAC will be assigned various projects related to healthcare access at Grady. The HEAC is responsible for effectively contacting patients for a variety of healthcare related tasks including but not limited to scheduling, information gathering and follow-up. The HEAC will also be involved in projects and duties that improve community access to healthcare. Most of the work is remote but there may be times when face to face is needed. The HEAC will report to the Chief Health Equity Officer. The HEAC will work as needed.

Essential Position Responsibilities

* Make daily calls to patients as it relates to the project on that day.

* Make approximately 30 calls daily.

* Attend weekly meetings with the Office of Health Equity

* Document and track patient interactions in a daily Excel workbook

* Input patient interaction and referral data daily, tracking weekly metrics

* Provide weekly report to the Office of Health Equity

* May require some participation in community events

* Establish a professional working relationship with other leaders at Grady

* Establish a relationship with others who are on the project team.

Education

* High School Diploma

Qualifications

* Basic knowledge of Microsoft Office and Microsoft Excel

* Ability to work remotely in a quiet space

* Effective communicator

* Ability to work at least 10 hours per week
